A few years ago I was flying out to Hawaii and told the security guys that I was diabetic and had a pack with drugs and syringes and stuff in it and they actually asked me if it was ok if they had a look. I didn't have a doctor's letter or anything. They seemed quite hesitant to impose which I thought was strange.
I think it basically depends on which security guy you get. Personally I'd be more comfortable if they went over my stuff with a fine tooth comb so I know the other passengers are getting the same treatment and the flight is more likely to be safe.
Sorry for rambling but yes I was told by a couple of airport staff that it's best to get a letter from a doc - syringes are always going to be an issue (and should be) since 9/11.
